Before you start

  • Confirm the CMRA delivery address and your assigned PMB with staff. Have your actual home address and business details ready.
  • Prepare two distinct, current acceptable IDs for each applicant and any separate pickup designee: one photo ID and one address ID, with copies of both.
  • Decide who receives mail, whether a separate person may collect it, and where physical mail or email scans will go.
  • Arrange the permitted signature witnessing or notary process before signing. Do not enter identity details into a public example or an assistant.

Client / applicant

Complete your applicable sections and supply both IDs. Leave section 14 and the notarial certificate to the appropriate professional.

CMRA staff

Confirm sections 1–2, verify identities, recipients, destinations and signature evidence, and complete the CMRA-only fields as applicable. Review before activation; submission is not automatic approval.

Notary

On the notary route, complete only the page 2 notarial certificate/seal. Notarization does not make the notary a pickup designee or CMRA employee.

June 2024 section responsibilities
Section 1CMRA staffStaff supply or confirm the date PMB opened/closed. Do not invent an opening date or enter a closing date for a new mailbox.
Section 2CMRA staffStaff supply or confirm the official delivery street address, assigned PMB number, city, state and ZIP + 4. Do not use an unassigned or example PMB.
Section 3ApplicantSelect Business/Organization Use or Residential/Personal Use. This choice determines sections 7 and 12 and separate-form requirements.
Section 4ApplicantEnter your legal name, contact details and actual street home address, including apartment if applicable. Your home address must match the address ID in section 9; do not substitute the CMRA address. If court-ordered protected, follow 4k and attach the court order securely.
Section 5, 10, 11Applicant · only if designating a pickup personComplete only for a separate individual you authorize to pick up mail. Enter that person's legal name, contacts and actual home address in 5, photo ID in 10 and separate address ID in 11; attach both ID copies. The address in 5 must match 11. A CMRA employee acting for the agency or a notary witnessing your signature is not this pickup designee.
Section 6Applicant · conditionalComplete when PMB mail will be transferred, mailed, shipped or emailed to another address. Include the applicable destination/contact details; email scans count too. Confirm planned destinations with staff, even if no physical forwarding is requested.
Section 7Business/organization applicantComplete business name, type, street address, telephone and place of registration. Place of registration means county/state for domestic entities or country for foreign entities (footnote 8). An officer must sign section 13 and supply their title (footnote 14).
Section 8, 9ApplicantRecord one acceptable photo ID in 8 and a separate acceptable address ID in 9. Attach copies of both. The address ID must contain the home address entered in 9 and match section 4.
Section 12Applicant · additional recipientsBusiness/organization: list members receiving mail; each must present two valid IDs to USPS upon request. Residential/personal: a parent or guardian may list a minor's name without the minor's ID. Each adult residential user needs a separate Form 1583; listing an adult here does not replace their form.
Section 13Applicant / business officerSign and date using the witnessed or acknowledged process below. A business officer also supplies their title. An ordinary typed signature or uploading a signed PDF alone is not sufficient.
Section 14CMRA / authorized employee onlyClient: leave blank. Only the CMRA agent or authorized employee completes this signature/date field when required for the witnessing route. A notary does not sign here as the CMRA or replace the CMRA's role; staff confirm applicability under the official instructions.
Section Page 2 notarial certificate / sealNotary only · notary routeOnly the commissioned notary completes the certificate, signature, commission expiry and seal when the notary route is used. The notary is not your mail-pickup designee and the notarial certificate is separate from section 14.

Two IDs — not the same document twice

Use the official choices below (8e/10e and 9g/11g). Footnote 9 calls for a government-issued photo ID; staff must verify the chosen document against USPS requirements. A driver's/nondriver's ID may serve as photo OR address ID, never both for the same person. Address evidence must contain the actual home address. Utility bills, credit cards and Social Security cards are not listed accepted options on this revision.

Signature in section 13 — choose an authorized route

Sign or confirm your signature in the physical presence, or virtual presence with real-time audio AND video, of the CMRA Agent or its authorized employee. Alternatively, acknowledge your signature in that physical or real-time audio/video presence of a notary commissioned in a U.S. state, territory, possession or the District of Columbia.

A typed signature, prerecorded video, or uploaded PDF alone does not meet this witnessing/acknowledgment requirement. Arrange the process with staff; this guide does not provide a signing or remote-notary service.

The form's page 2 note says the agent completes 14a/14b “if necessary (i.e., if the agent is the witness).” Clients leave 14 blank; staff determine the appropriate completion. A notary completes the separate page 2 certificate, not section 14.
